I don't know if you have to have proof of citizenship in order to buy healthcare, or if you are subsidized. I would imagine you would only have to prove that your income was below a certain level. I really don't know much about the current system. Aren't subsidies tied into Medicaid too? I remember there was a flap when certain states refused to expand medicaid.
As far as the $100M, could it be for counter-drug operations? You know, if you start doing reseach, it might surprise you how much different countries are getting. "Foreign Assistance" is a very broad term for a lot of different things. Biodiversity, a medical program, helping them buy weaponry from our companies, counter-drug operations, nation development (bringing water to small towns), land mine removal, training. So before you get your knickers in a knot too tight, some of those are actually good programs. And in the overall scheme of things, $100M is not a lot.
