Don't worry, the engineers of this grand plan will come up with a "solution" to the doctor dilemma - how about this:
No "doctor" will be able to practice medicine without proper FEDERAL credentials - state and national MD groups will become anachronisms...and there will be fines/imprisonment for those who are found to be practicing without "your papers."
Problem solved, no way out, total control, welcome to the third world.
